NRC DNP Telehealth LLC offers telehealth services for adults and geriatrics who need primary care at home. We also provide comprehensive diabetes education to those who are diagnosed with this disease. You no longer have to leave your house and visit the nearest clinic for a health check. Simply dial our phone number, and we’ll provide you with the diagnosis and treatment you need.

Additional Services
- Remote Physiologic Monitoring (RPM): RPM is using mobile devices and technology to obtain automatically generated patient data (blood pressure, blood glucose, heart rate, weight, and O2 saturation), which is then sent to the clinician to review.
- Cognitive Assessment screening (Cognitrax): remote computerized neuropsychological testing used by clinicians to evaluate and manage the neurocognitive state of a patient (ex: dementia). Across the lifetime, serial testing allows ongoing assessments of a patient’s condition, disease progression, or clinical outcome. Cognitrax's primary function is to (measure) serve as an instrument, to detect early cognitive impairments and identify symptoms, behaviors and comorbidities that can impact health. Secondly, is to augment data generated from the clinical history, the examination and other tests (evaluate). Thirdly, to longitudinally track cognitive function (manage & monitor).
